The comprehensive community health improvement process includes two major phases: a Community Health Assessment (CHA) and a Community Health Improvement Plan (CHIP). The CHA and CHIP are rooted in the principles of health equity and community engagement. They help identify and address priority health issues in a community.
Since 1993, Wisconsin law has required local health departments, including Door County Public Health, to assess and create a plan to address health issues affecting their residents. Wisconsin Statute Chapter 251, Section 05 details the requirement that all local health departments must complete the CHA and CHIP at least every five years. We work closely with our hospital partner, Door County Medical Center, to meet their IRS requirement as a non-profit healthcare system to complete a Community Health Needs Assessment (CHNA) & Implementation Strategy (IS) at least every three years.
New for 2025, we have rebranded our CHA and CHIP work as Healthy Door County. Healthy Door County is a diverse representation of multi-sector community leaders collaboratively working to improve the health of the community.
