Door County Economic Support is part of the Bay Lake Consortium


Who Do We Serve?

Eligible recipients residing in Door County who fall within the guidelines of Federal and State regulations. Low income individuals or families with basic living and health care needs.

The following can be done online at access.wi.gov and/or by phone 1-888-794-5747:

  • Apply for benefits
  • Report changes

  • Make an appointment


There are four ways to apply for  FoodShare, Medical Assistance, BadgerCare, and/or WI Shares Childcare Assistance:

 

1. Online at access.wisconsin.gov

2. By Telephone by calling the Bay Lake Consortium 1-888-794-5747 (toll free) 

  • Hours: Mondays, Tuesdays, and Wednesdays: 8 am-4:15 pm, Thursdays: 9 am-4:15 pm, Fridays: 8 am-11 am

3. In-Person at the Door County Health & Human Services lobby, 421 Nebraska Street, 2nd Floor, Sturgeon Bay, WI 54235. 

  • Some brief paperwork will need to be completed, and then a  staff member will set up an appointment to meet with an eligibility worker.
  • Hours: Monday-Thursday 7:00am-4:30pm and Friday 7 am-11 am. In-person Economic Support Services will no longer be available after 11 am on Fridays. Lobby will remain open to drop off documents.

4. By Mail: A paper application can be requested by calling (888) 794-5747. 

  • The paper application can be returned via fax to (855) 293-1822 or via mail to: CDPU; PO Box 5234, Janesville, WI 53547-5234.
